Head of Health and Social Care - 6 months fixed term

This role is an exciting new development for Sutton Health and Care (SHC) as part of Epsom and St. Helier University Hospitals NHS Trust (ESTH) and the London Borough of Sutton (LBS) and aligns to the work of the Integrated Care Parnership Board (ICP) that Sutton as a PLACE within the SWL Integrated Care System have committed to.

The Head of Health and Social Care is a joint appointment (hosted by ESTH) between SHC and LBS. This role is a strategic leader across the systems of the NHS and social care, a senior operational manager and has a key transformational aspect to this portfolio of work.

The post holder will manage staff from social care and health care, managing the social work and community nursing teams for the Wallington neighbourhood. The role for both includes staffing management, budget management as well as being responsible for service performance, KPIs and achievement of population health outcomes and social care KPIs in conjunction with our key partners.

The role also has a critical senior lead role in the Wallington Integrated Service Transformation Programme and includes delivery on an integrated team concept, joint front door, triage and assessment process as well as the ongoing functionality of the day-to-day core business of this integrated team. This is an exciting programme of work linked to the ICP objectives and linked to the Integration of Community around Primary Care Networks (PCNs).

As an expectation, the candidate must, therefore, have substantial experience as a senior manager in the health and social care field, having managed multi agency and multi professional staff and have good experience of multi-complex transformational project work.
