Affinity Trust have won the contract to provide support into individual’s homes as part of our Transforming Care programme. This is new exciting project funded by NHS England and developed by Choice Support.

We'll be supporting people on a 3:1, 2:1 and 1:1 basis, depending on the individual need.

About The Role

  • You’ll support people with all aspects of daily living, including cooking, cleaning, shopping, meal prep, minimal personal care, planning routines and managing their days etc.
  • Work with individuals to reduce the frequency and intensity of behaviours that may challenge
  • Be involved in the transition plans for individuals moving into the location
  • Help the people you’re supporting become more involved in their communities
  • Support people socially and to enjoy hobbies and activities, like swimming or cycling
  • Assist with volunteering/employment opportunities
  • Support with holidays, trips and days out and more

Shift Patterns

Shifts are over a 7 day rota including weekends and bank holidays.

  • 08:00 - 22:00
  • 08:00 - 20:00
  • 22:00 - 08:00
  • 20:00 - 08:00

07:30 - 15:00 & 14:30 - 22:00 hours also available.
