Assistant Practitioner/Pathway Facilitator
An exiting and challenging position has arisen with Walsall Community Nursing Service.
We are looking for highly motivated support workers with experience of community nursing, to work within our MDT to support safe discharge from hospital and prevent unnecessary hospital admissions.
You must be an excellent communicator and have the ability to work autonomously, whilst also being part of a team working with an integrated approach to care.
The successful applicants will need to be:
• NVQ L3 or equivalent qualification and/or relevant experience
• Have a foundation degree in Health and Social Care or be willing to undertake or have a equivalent qualification. You must be willing to travel frequently within the working day throughout the borough.
• Please note that at risk NHS employees will be given preference for this position.
