As a Service Development Officer - for the Integrated Urgent Care (IUC) Service you will be the first point of contact for Commissioners, Local CAS Providers (GP Out of Hours, Mental Health etc.), Social Care and Third Sector (Voluntary Organisations) within a Sustainability and Transformation Partnership/Integrated Care System (STP/ICS) footprint to support the development of urgent and emergency patient pathways to improve overall patient outcomes and experiences.

To deliver this vision we wish to appoint a Service Development Officer to work in the dynamic, fast-paced IUC service across Yorkshire and the Humber with new challenges and developments every day to ensure the best outcomes for patients accessing the IUC service.

In line with our Trust’s strategic ambitions to ensure patients and communities experience fully joined-up care responsive to their needs and excellent outcomes, we deliver IUC through collaboration with primary care colleagues, other providers and Commissioners. We will lead the way in transforming from an ‘assess and refer’ signposting service to a ‘consult and complete’ service, where patient needs are resolved through advice, a prescription, or appointment.
