Summary


An exciting opportunity has arisen for Assistant Practitioners to join Adult Community Therapy around the Sleaford area. As a therapy focussed Assistant Practitioner you will focus on transitional care beds within care home settings to co-ordinate the care of patients who have been admitted into these beds whilst working alongside registered therapists and therapy assistants to ensure their rehab and holistic needs are being met. However, this role could also include completing assessments with people within their own homes to identify and provide therapy interventions to help a person achieve. To apply for this position, you must have a foundation degree or equivalent.

The main role of this position is to support patients who have been admitted into transitional care beds within a care home setting. You will need to deliver high quality patient centred care whilst ensuring you are maintaining patient flow and aiming for the person to return home as soon as possible. You will be the main contact for the patient and their family and develop close working relationships with care home staff, therefore you will need to demonstrate good communication skills. You will complete holistic assessments of the patients who come into our care and support admission and discharge plans. However, as capacity allows, there will be opportunities to complete assessments in the community with people in their own homes. You will need to have an ability to travel, possibly around the county, but more locally around the Sleaford area due the community nature of this position.

The Adult Community Therapy Team (ACT) within Lincolnshire Community Health Service (LCHS) is made up of Physiotherapist, Occupational Therapist, Assistant Practitioners, Senior Therapy Assistants and Therapy Assistants. We receive referrals from other health and social care professionals for people who are have difficulty in managing their activities of daily living. We provide a rehabilitation service to these people to support them to live as independently as possible whilst working to their set goals. We offer regular clinical supervision, appraisals and in-service training to team members.
