Camden Integrated Practice Unit (IPU) is an national recognized award winning service. We have a vacancy for part time Diabetes Specialist Dietitian at Band 7 to join our amazing community team of diabetes specialists nurses, consultant , podiatrist, psychologist and dietitian.
The Camden Diabetes Integrated Practice Unit (IPU) is a nationally recognised award-winning service. It is a diverse multi-disciplinary team that works in primary, secondary, and tertiary care to design care plans that meet the needs of people with Diabetes. The team serves patients registered with a GP in the Camden borough. There are over 30 GP surgeries and 7 Primary Care Networks (PCN). Our work is divided into 2 localities i.e. North and South with clinics based at St Pancras Hospital and Belsize Priory Health Centre. Diabetes can have a significant impact on the quality of someone life and the financial implications on the NHS. The Team aims to optimise health and wellbeing, reducing complications and prevent unscheduled hospital visits.
The post holder will be an expert dietetic practitioner as a Diabetes Specialist. They will work autonomously without direct supervision, to deliver evidence-based care, education, and information to people with diabetes and their families referred to the Camden Intermediate Diabetes Service (IPU).
They will demonstrate and understanding and commitment to providing all patients with Diabetes the help they need to identify what is important to them, what they would like to achieve and how they plan to achieve it. Post holder will demonstrate that they have the attitudes and behaviours required to support people to make and sustain lifestyle changes.
The post holder will provide specialist clinics, structured education programs for adults with diabetes and training programs for health care professions.
