Physical Healthcare Facilitator BAND 3

£23,071 to £24,499 inclusive of allowances

Full time position Monday to Friday 08:00-16:00 hours.

We are looking for an enthusiastic and capable Band 3 Physical Healthcare Facilitator (PHCF) to work in the Physical Health Centre (PHC) providing high quality care to patients with physical health care needs. You will be a key member of the PHC team and the primary functions of the role are as follows: ECG Recording / Basic wound care / Supporting the Practice Nurses & Allied Health professionals / Administrative duties / Stock maintenance & control and relevant IT skills.

You will be working within a supportive multidisciplinary physical healthcare team, including: a GP, Senior Nurse Practitioner and Practice nurses. Attending allied professionals include: Physiotherapist, Dietician, Dentist and Podiatrist. The PHC is also supported by a range of visiting speciality consultants from Frimley Park Hospital and we also participate in the national screening programmes.

To be considered for this role you will need to be enthusiastic, forward thinking and emotionally resilient. You will also need to possess the following skills and experience:

You will undertake a thorough hospital induction followed by a secondary induction which is tailored to the PHC. We will also provide you with first class clinical supervision and actively encourage both personal and professional development by offering a wide range of relevant courses / study days, annual mandatory training and opportunities for University based training.
