Health and Social Care integration is moving at a pace in Oldham and we are looking for an innovative proactive Band 5 Community Nurse who are at the beginning of the integration journey.

Do you have an innate passion for supporting community services to transform in line with the governments Health & Social Care agenda? We are not just looking for ‘staff’; we are looking for someone who wants to use both their clinical skills and personality to make a BIG difference in a challenging but rewarding setting.

We are looking for someone to deliver high standards of care/service to users/clients in their own home or in a clinic setting as part of the health and social care team, working as an autonomous Practitioner.

Successful candidates must hold a full, clean UK driving licence.
