An exciting new opportunity has risen in North London for a large local authority, within the Transitions Team in the adults services. This role has arisen due to an increase in work within this authority.
Your new role
For the purposes of Haringey Learning Disability Partnership, we use the term 'Transition' to mean work with service users' who are aged 16-25 and caseloads of Social Workers working with young people in transition would reflect this. Whilst we do not take on responsibility for cases prior to their 18th birthday, social workers will be required to work with young people and their families prior to this to ensure our assessments of eligibility and guidance through support planning is timely, relevant and accurate. You will be expected to engage with education, employment, accommodation, and other support services required by a young person and their families. This will also include contributing to Child Protection procedures, supporting young people being discharged from hospitals.
As a Transition Social Worker, you are expected to work with a range of other council departments and external agencies to ensure that there is a coordinated person-centred planning process between Children's and Adult's services. These agencies could include Special educational needs department, Children with Disability Team, Young people's service (Leaving Care Team), Locality children's service, Mainstream adult social care, Local schools and colleges, Housing, Community Safety Teams and others as appropriate.
Your role will entail some sub-sect over-lap, and therefore these may include carers assessments, Care Act Assessments, Continuing Healthcare Checklist and Decision Support Tools, Support Planning, Duty, and Mental Capacity Assessments and Court Protection Applications.
What you'll need to succeed
In order to be a successful candidate for this role, firstly you will need a degree in Social Work. You will need to know the relevant legislation and best practice in relation to preparing for Adulthood, Family and Children's Act as well as Special Needs Frameworks. You will also need to be able to engage in Family and Children services frameworks, such as Child Protection, EHCP reviews and Child in Need reviews. As a Transitions Social Worker, you will need sound knowledge about the service and what they offer such as transition fairs, The Local Offer, Leaflets and documents about how Adult Services support people.
