If you are passionate about providing high quality nursing care to people in their own homes and want to support individuals to remain in their preferred place of care, preventing unplanned admissions, then the Out of Hours Nursing Team within Community Health and Social Care is for you.
The team currently provides a service from 16:00 – 08.00hrs worked over 2 shift patterns 16:00 – 00:00hrs and 22:30 – 08:00hrs due to Covid-10. This is temporary and is under review so may revert to the previous shift patterns, 14.30 – 22.30hrs, 18.00 – 02.00hrs and 22.30 – 08.00hrs. The team supports the community population across Exeter, East and Mid Devon; we are currently based in Ottery St Mary.
We are currently looking to further develop our out of hours service utilising skills to deliver responsive, timely, quality care to meet the needs of our population. At this exciting time, we are seeking to recruit enthusiastic, experienced nurses to the Band 6 role of Community Nurse Specialist. You will be able to demonstrate your enthusiasm and commitment to community nursing, highly developed clinical skills to share as the clinical expert and be able to evidence your leadership skills/experience to support junior team members. The post-holder will be supported by the Team Manager and a Senior Nurse.
Our Out of Hours Health and Social Care Community Team, consists of nurses and healthcare assistants. The team are developing closer links with the day community nurse teams to provide an element of planned care and to support the needs of our populations around the clock, also undertaking responsive holistic nursing and social care as required. The caseload covers a broad range including end of life care, admission avoidance and support for timely hospital discharge. The team also provide clinical support for our Urgent Community Response support workers between the hours of 5pm – 11pm.
This post provides a huge opportunity to harness the enthusiasm of the current team to develop the service further, enhancing the nursing skills of junior staff to broaden the provision available to patients.
Applicants will be required to commit to all shift patterns within the team and be comfortable with driving, sometimes long distances across the geography. Working hours will be 22.5 hour per week.
