As the Healthier Together plans continue across Greater Manchester the Oldham Care Organisation has a full accident and emergency department and offers a comprehensive range of acute, general and specialist surgical services. The site also includes a dedicated Women’s and Children’s unit, offering one of only three Tertiary maternity units in the area. It is also home to the Alliance’s state of the art pathology lab providing a range of pathology services across the region. Recently the Royal Oldham has received feedback from the CQC recognising improvements and the dedication of the teams in their commitment to providing exceptional care.

The Role within Our Team

An opportunity has arisen for an experienced Team Leader for our Discharge Co-Ordinator and Patient Flow Tracker Team within the Integrated Discharge Team, based at The Royal Oldham Hospital.

You will have proven team leadership experience and be able to provide clinical expertise and co-ordination to facilitate complex discharge planning for the team, including discharges outside of our local area.

You will be an experienced nurse or health professional with substantial experience in complex discharge planning. You are able to demonstrate first class communication skills and an ability to use your negotiation and problem solving skills to diplomatically unpick multi-faceted issues whilst also carrying a fast-paced caseload, requiring a high degree of flexibility and prioritisation.

You will play a pivotal role in developing the Integrated Discharge Team, and multidisciplinary ward teams (MDT) to maintain safe, effective and efficient discharge pathways to reduce delays and maintain patient flow throughout the hospital by driving down delays in transfers of care, bed occupancy and lengths of stay through an integrated health and social care approach.

You will work flexibly and proactively with all hospital and non-hospital based teams to support patient flow and efficient discharge planning, being instrumental in developing appropriate and efficient patient focused pathways. You will be required to demonstrate your creativity by developing new initiatives and approaches to enhance patient flow and experience.

As a Nursing/Health Professional role within the organisation, there is an expectation that you will fully participate in the clinical and professional leadership and delivery of high quality patient care.

You will be able to work contracted hours over a 7 day period.
