Summary


Health Care Assistant in the Home First Team, part of the Community Independence Service (CIS) in Hammersmith and Fulham West London NHS Trust.

Band 3

We are looking for an enthusiastic and motivated Health Care Assistant to provide care to patients in the community setting in Hammersmith and Fulham Borough.

You will be working for the CIS Home First Team, which provides a rapid response service for people who need urgent help with an acute illness in the community, often immediately after a hospital admission. Alongside team members you will help provide rehabilitation and re-enablement to assist patients to achieve functional goals and improve their independence, and carry out and re-learn daily activities and practical tasks after an injury or illness.

As a Health Care Assistant you will be expected to provide support to our patients in a wide range of community environments, working autonomously and collaboratively in supporting the integrated multidisciplinary team in the delivery of excellent patient care. You will be responsible for the holistic, comprehensive care of complex community patients, delivering clinical care delegated by a registered professional and contributing to the evaluation of person centred care plans.

You will have excellent communication skills, IT skills, and the ability to be adaptable, flexible and reliable. You will have good organisation skills and the ability to prioritise your workload effectively. Experience of care in a community setting would be an advantage but is not essential.

We have a strong focus on developing our workforce with comprehensive statutory and mandatory training alongside higher level learning in partnership with local Higher Education Institutions and Health Education England.

Home First is a shared service between Central and North West London NHS Trust and West London NHS Trust covering Hammersmith & Fulham, Kensington & Chelsea and Westminster Boroughs, although your main base will be Hammersmith and Fulham.

Please note Home First isnota mental health service, vast majority of our patients need support with functional activities.
