Summary

North Lincolnshire Clinical Commissioning Group (CCG) is looking for an experienced, enthusiastic and motivated Continuing Healthcare Team Lead to join our Continuing Healthcare team within the Nursing and Quality Directorate.

North Lincolnshire Clinical Commissioning Group (NLCCG) covers a population of almost 172,000, based around the Scunthorpe urban area with a large rural area stretching from Epworth and Crowle in the West, to Barton upon Humber and Immingham in the east. There are 19 GP practices within its boundaries all of whom are members of the Safecare GP Federation.

North Lincolnshire Clinical Commissioning Group is responsible for commissioning high quality health and adult social care services for the population of North Lincolnshire. We work closely with providers of health services, local government, regulatory bodies, voluntary and carer groups, patient representatives and the general public to achieve the best health and care outcomes possible and to ensure high quality, safe and effective services.

We are seeking a self-motivated individual with excellent leadership qualities. We are particularly interested in applications from Registered Mental Health Nurse / Learning Disability Nurses.

The successful applicant will lead and manage the Continuing Healthcare (CHC) assessors / reviewer within the Clinical Commissioning Group (CCG) and to work in partnership with key stakeholders. You will responsible for the team’s compliance against the National Continuing Healthcare Frameworks. You will ensure high quality standards run though all aspects of the team’s delivery, overseeing the procurement of commissioned care in addition to ensuring that quality, safety and value for money through person centred care.

You will need excellent organisational and communication/interpersonal skills. Whilst having leadership experience you will be looking to further develop your management and leadership experience.

The successful candidate will be fully supported by the senior team and be provided with a bespoke induction plan in order to ensure success within the role.
