An exciting opportunity has arisen for a Specialist Renal Social Worker to join the Renal team based at Swansea Bay University Health Board. The Renal team cover a wide area including Swansea, Neath Port Talbot, Bridgend, Carmarthenshire, Pembrokeshire and Ceredigion.

The successful candidate will provide a highly specialised social work service, which includes assessment and Social Work interventions with Renal patients and their families. The service is to be provided to a large geographical area which will include the post holder visiting designated wards, outpatient clinics, transplant clinics and dialysis units.

There is a strong emphasis on teamwork, as you work as part of a multidisciplinary team to support people with complex Renal conditions.

As a Specialist Renal Social Worker, you will have previous experience from the NHS or private care as you manage a complex caseload.

As part of this role the successful candidate will:
• Facilitate access and provide advice on welfare rights, housing, advocacy services and associated support services.
• Be responsible for evaluating the Social Work Service provided to Renal patients.
• Takes a lead in the development, delivery and evaluation of the post, and service delivery.
• Identifies needs, provides and manages Social Work care for Renal patients and their families.
