We are looking for a vibrant and dynamic person to join our award winning, pioneering team in this exciting role.

The post holder will develop, implement and evaluate a seamless specialist service, ensuring that patients receive the highest standard of clinical care. The focus of the role is to lead the clinical assessment of patients benefiting from advanced discharge or admission avoidance through care provided in their own home, by the GSTT@Home multi-disciplinary team.

The successful candidate will demonstrate a high level of expertise in this field providing advice, education and support to health care professionals and patients and will actively undertake clinical audits.

They will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, as well as policies, protocols, guidelines and service developments. This role will predominantly be based in the community but will rotate into the roles of Triage Nurse and In-Reach Nurse located within Guys and St Thomas’ Hospital for patients in their own environment within Lambeth and Southwark.

The advanced nurse practitioner and triage role will assess electronic and telephone referrals and assess acutely unwell patients at home using advanced assessment skills to avoid hospital admission unnecessarily.

We are looking for an innovative person who is keen to both teach within and develop the service.

This post is a 7 day role and provides a service within the hours of 0800-2300.
