Currently, acute hospitals within Cambridgeshire and Peterborough are being prevented from performing their vital functions because their beds are filled by almost well people who are not able to return to their own homes – however with CPFT Intermediate Care they can.

We are excited to announce an expansion of our existing Intermediate Care Team. We are recruiting for an assistant care- co-ordinator to support care provision in our team in East Cambs and Fenland.
The vision of the service is to provide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital; early assessment for care needs and avoidance of hospital admission.
We will provide the necessary support to ensure your development within your role. We are keen to support and develop our staff through a clear career pathway and encourage promotional opportunities within the Trust.

We are looking for full time staff who are able to work flexible shifts patterns within our core service hours. Our operating hours are 7am to 10pm, 7 days a week.
