A Vacancy at Central and North West London NHS Foundation Trust.
The Community Independence Service is looking for someone to join the Community Independence Service (CIS) Leadership Team, with responsibility for leading the Rapid Response and Home First team which is based in Westminster. We strongly encourage applications from Nurses and Paramedics so to allow a broad variety of candidates to select from.
Our teams are community based and as the lead for this pathway you will be expected to work within the community, be flexible with your working time and attitude and lead by example by having excellent clinical abilities as well as management and leadership skills that you wish to develop.
The Community Independence Service (CIS) is led by CNWL and partners. We provide health and social care support to people from Hammersmith & Fulham, Kensington & Chelsea, and Westminster in their own homes when they are unwell, recovering from illness or at risk of unnecessary admission to hospital. CIS covers all 3 boroughs and within each borough we have the following teams:
- Community Rehabilitation
- Rapid Response
- Home First (Discharge 2 Assess)
- Reablement

Successful candidates will be of a health or social care background with experience of leading community, acute or rapid access services and an understanding of the changing face of health care. You will enjoy the rewards of working with a client group for whom the teams will make a significant difference every day, and will experience the strong motivation of being part of a supportive multi-agency effort .
There are numerous opportunities for development within CIS and CNWL. The organization has a very established catalogue of learning and development opportunities, and a strong supervision and appraisal culture. They are actively developing an approach to Quality Improvement and working closely with the Institute of Healthcare Improvement as part of a 3-year programme. The post-holder will be able to access training and lead on projects using QI methodology to support and develop the team.
