An exciting opportunity has arisen for an Advanced Clinical Practitioner to join the Integrated Health and Social care team in the Vale PCN hub based in Shaftesbury in North Dorset. The post holder will work together with Primary Care, Community Services, Social Services, Voluntary Sector, etc to provide enhanced and pro-active care as part of a frailty team.
We are keen to recruit a Practitioner coming from a variety of professional backgrounds with the ability to provide quality and personalised care plan for patients with complex needs. The community hubs work as a multidisciplinary model and consist of Medical Consultants, GP's, Advanced Practitioners, Community Matrons, Medicine Management, Health & Social Care Coordinators, Adult Social Care, etc.
The post holder’s remit is to provide proactive and reactive care to prevent unnecessary hospital admissions by supporting individuals to remain in their own homes by undertaking holistic and generalist assessments at the point of need. You will assist individuals and their carers to proactively anticipate their future health needs, support self-care, develop appropriate plans, manage deterioration via early intervention and work collaboratively with the wider MDT to promote improved outcomes and the management of frailty.
This exciting and challenging role offers the post holder the opportunity to provide high quality patient focused care in the community. The role supports the Advanced Clinical Practitioner in the community through innovation, clinical skills and diagnosis, treatment, review and referral.
The post holder will be expected to engage and act within a range of services delivered in the hubs including inpatient services, Virtual Wards, rapid access services and care closer to home.
Successful applicants must have a broad experience in community and/or urgent care at a senior level. You must be motivated and enthusiastic and have proven experience of working as an autonomous practitioner.
Ideally applicants will have the following qualifications however if you don’t have all of these but have the right attitude then we can support you to develop further in line with professional advancing practice:
• • Current registration with NMC/HCPC
• Relevant professional health degree
• Evidence of recent CPD
• Post Graduate Diploma in Advanced Practice or specialist practice
The post holder will be expected to cover 8:30 to 18:30 Monday to Sunday.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of £40 and the Trust will recover this cost in instalments from your first 3 salary payments.
