We are pleased to announce an exciting vacancy for a full-time Band 3 Therapy Assistant to support our Community Interim Care Team. We aim to provide early supported discharge for elderly patients from Bournemouth acute hospital and we have also been facilitating patients to receive social care assessments out of hospital from interim beds.

This is an opportunity to join our community service which is currently assessing for and bridging for care packages in the community. The role will include 7 day working and shift work, (earlies, lates and long days) as our community service operates between 7 a.m. and 10 p.m. seven days a week. Our team is based at both Bournemouth and Christchurch hospital and staff will be expected to be able to report in to both bases. We are able to consider full time and part time hours. The role will involve community work and for this reason a car owner/ driver is essential. We support patients across South Dorset and West Hampshire and staff will be paid for time and mileage incurred during work.

The key responsibilities of the position will be to follow the care plans and goal plans of our patients in their own homes after discharge from hospital under the support of the clinicians within the team. We aim to maximise independence and reduce the reliance on care during those early days after discharge from hospital and for this reason clear feedback on goals achieved is essential to enable us to reduce dependency and create further capacity within our team. Our work in the community involves supporting patients with personal care and medications therefore we are looking for staff with hands-on experience of health and social care. You may also be required to support the team in the work within the hospital, screening patients suitable for interim beds or community care by gathering information and speaking to patients and their families and there are some administrative aspects of the role we require you to complete. You would also be required to support the patients within the interim beds by carrying out their rehabilitation programme where appropriate as set by the therapists within the team.

You will participate in a competency based training and assessment programme to develop the skills required for this exciting role, therefore you must demonstrate a high level of motivation for continued personal/professional learning and development. We hope to develop opportunities to rotate within the hospital therapy teams in the coming future to provide further opportunities for personal and professional development.

The Interim Care Team at the Royal Bournemouth Hospital are an extremely dynamic, innovative and supportive multidisciplinary team looking to improve and integrate health and social care for older people in Dorset and its surrounding areas. We have been successful in facilitating patients to return to their own homes and therefore reduce their hospital stay which has been demonstrated to significantly impact health and well being outcomes.
