An exciting opportunity has arisen in South East Essex for a Band 6 Registered Nurse to join the SWIFT Admission Avoidance Team.
SWIFT is a community admission avoidance team available 7 days a week that helps patients to stay at home when they are feeling unwell rather than be transferred to hospital. The SWIFT team provides specialist nurse and occupational therapy led care in patients own homes. They visit patients within two hours of receiving a referral from a healthcare professional to stabilise their immediate health needs and plan on going care and treatment as appropriate.
The Band 6 Team Leader will work to support the delivery of crisis care and support in patients own homes to support local admission avoidance pathways, through being responsible for assessing, planning and providing care to patients with acute health care needs.
