The Hospital Integrated Discharge team consist of qualified social workers, community assessment officer, qualified nurses, occupational therapist, nursing assistances and housing officer who all work closely with the multi-disciplinary teams based on the individual wards within a busy hospital setting.



We are looking to recruit five full time, preferably level 3 experienced, social workers to come and join our dedicated team of staff in providing a social care service to patients in Salford Royal Hospital.



We have two full time permanent social work positions on offer and 3 full time temporary posts to cover maternity leave, the temporary posts will be for more than 4 but less than 12 months duration (duration may change in line with the time taken up by the recruitment process).



You will be responsible for carrying out social work duties, requiring the highest levels of skills, knowledge and professional expertise.



The team supports the wards to facilitate safe and timely discharges of patients from the hospital. To achieve this they use a wide variety of personalized support, assist people to promote their independence, support people with complex needs and often changed situations to ensure they have the maximum level of choice and control available to them and promotes the principles of The Care Act and MCA.



In order to provide a comprehensive the team operate a 7 day service which includes working extended hours during the week to 7pm on a rota basis to facilitate timely hospital discharge planning and safe discharges from hospital.
