An opportunity has arisen to lead the Reablement Team at Yeovil District Hospital NHS Foundation Trust on a fixed term basis to cover maternity leave, end date to be confirmed. This role is a Job Share.
We are looking for an individual who wants to support patients in reaching their rehabilitation potential by working alongside Physiotherapists and Occupational Therapists on the Home First pathways at Cooksons Court and Sherborne House in Yeovil and occasionally in their own home.
The successful candidate will have an understanding of the current challenges for Health and Social care and be able to demonstrate leadership of Physiotherapists, Occupational Therapists, Rehabilitation Practitioners and Assistants. This role leads a team across the Home First pathways, including sites in Yeovil and Glastonbury as well as people's home. Weekend and Bank Holiday working is expected and this may include any site, including occasionally wards at Yeovil District Hospital. They will have excellent communication skills and be able to liaise with a number of internal and external agencies.
