The newly established Crisis Resolution Home Treatment Team (CRHT) is looking for two Discharge Coordinators to act as a key liaison person between the inpatient units and the CRHT. The post holders will be expected to work closely with inpatient clinicians in providing clinical advice and expertise regarding the suitability of patients on the acute ward to be discharged and continue their care and treatment in the community.

The post holders will be responsible for the effective coordination of patient discharges throughout the mental health units and will be expected to complete follows up in the community. They will work closely with inpatient clinicians and multi disciplinary team members to deliver a high quality, patient focussed service. The post holders must understand the challenges faced within these roles from both the inpatient units and positive risk taking in the community.

We are looking for two dynamic, proactive individuals who can take on the role of both an inpatient and a community practitioner. Individuals who have a good knowledge of social care and delayed discharge inpatient difficulties would be advantageous. As there is also an emphasis on community working, community mental health experience would also be beneficial. The ability to form networks and good relationships with other mental health services, statutory and voluntary sectors are key to these roles.

These roles require the working pattern of 9-5pm Monday to Friday.
