The Community Rehabilitation Service previously known as the Horizons Service operates in five localities across Kent to provide intensive rehabilitation and recovery focussed support to service users in their own homes.
The team use a holistic approach together with partners from Voluntary Sector agencies who provide additional Social Care and landlord/tenancy support to enable clients with complex mental health needs to manage more effectively in the community.
We currently have an exciting opportunity for an enthusiastic and motivated Occupational Therapist to join the established team in the Swale Locality. The successful applicant will care coordinate and manage a defined caseload of individuals with complex mental health problems.
The post also involves day to day coordination and supervision of support workers within the team to deliver comprehensive, individualised treatment and recovery plans which include supporting clients to maintain their living environment, and encouraging and supporting them to have meaningful roles and purpose within their local communities.
The post involves lone working, and includes 1 to 1 and specific group-work to provide individualised programmes of care to support clients to develop and enhance their skills and independent and promote recovery.
The successful candidate will be expected to work closely with partner organisations, to advocate for clients and to promote a collaborative and recovery orientated focus throughout. They will also be expected to work flexible hours between 08:00 and 20:00 according to the needs of the service, this will include some, evenings, weekends and public holidays.
Ideally the successful candidate will have previous experience of working in a community mental health setting, but we are prepared to suitable consider candidates with pre-registration experience and support them through their Preceptorship.

This post is subject to a six month probationary period for candidates external to the Kent and Medway NHS and Social Care Partnership Trust.
