An exciting opportunity has arisen within Cheshire and Wirral Partnership NHS Foundation Trust to join our Integrated Community Service, as an experienced Band 6 Community Nurse.
The post holder will be expected to lead by example in delivering effective Evidence Based Care to a defined demographic population with diverse health and social care needs.
You will possess excellent time management skills including the ability to prioritize and co-ordinate a defined caseload of patients. You will have a “can do” attitude and place patients at the centre of everything you do.
You will have the ability to work alongside a multidisciplinary team of Health and Social Care professionals, work autonomously to deliver high standards of evidence based care.
