A fantastic opportunity has arisen for a Clinical Pharmacist to join a highly regarded, forward-thinking GP training practice based in Reading. The Role
The successful Clinical Pharmacist will work as part of a collaborative primary care team, playing a key role in improving patient outcomes and supporting safe, effective prescribing.
Key responsibilities include:
• Conducting structured medication reviews and medicines reconciliation
• Supporting patients with long-term conditions and complex medication needs
• Managing repeat prescriptions and prescribing queries
• Providing clinical advice to GPs, nurses, and other healthcare professionals
• Supporting QOF, audits, and prescribing optimisation initiatives
• Delivering patient-facing clinics (face-to-face and telephone)

Requirements
• Qualified Pharmacist registered with the GPhC
• Experience working in primary care or GP practice preferred (training can be provided)
• Independent Prescriber qualification desirable (or willingness to work towards)
• Strong clinical knowledge and patient-focused approach
• Excellent communication and teamwork skills
