Job Summary
As a Care Coordinator, you’ll play a vital role in coordinating care services and ensuring high-quality support is delivered to our clients. You’ll work closely with carers, clients and families to keep everything running smoothly.
Duties
- Liaise with clients, their families, and care providers to assess needs and develop care plans
- Schedule and coordinate care services, including arranging appointments and visits
- Monitor and evaluate the effectiveness of care plans
- Maintain accurate records of client information and care plans
- Provide support and guidance to care providers
- Respond to inquiries and resolve issues related to care services
Experience
- Previous experience in an office environment is essential
- Excellent communication and interpersonal skills
- Strong organisational abilities with attention to detail
- Knowledge of health and social care services is advantageous
