Summary
DUTIES & RESPONSIBILITIES The GP Advisor will provide visible clinical leadership in relation to primary care integration, neighbourhood development and the wider strategic aims of Livewell Southwest. Internal Provide expert GP input to the Chief Operating Officer, Executive Team, clinical and operational leads on the design and implementation of Integrated Neighbourhood Teams. Work in partnership with community, mental health and operational leaders to shape multidisciplinary team models, integrated pathways and neighbourhood service redesign. Act as a trusted liaison with GP practices, Primary Care Networks and federations so neighbourhood development reflects frontline primary care realities and pressures. Support governance, clinical safety and transformation discussions across Livewell Southwest by bringing a general practice perspective to policy, service development and implementation. External Primary Care Networks, GP practices, federations and Local Medical Committee representatives. NHS Devon ICB, Local Care Partnerships, local authorities and VCSE partners involved in neighbourhood planning and delivery. Public health teams and wider system stakeholders contributing to population health and health inequalities work. System partners involved in emerging integrated care contracts, neighbourhood frameworks and collaborative service models. The GP Advisor will contribute to the strategic development of Livewell Southwest by advising on neighbourhood policy, integrated care design and the organisations position within emerging contractual models. This will include providing clinically informed advice to the Executive Team on opportunities, risks, delivery considerations and partnership priorities linked to integrated neighbourhood working. Management & Personal Development The GP Advisor will provide expert clinical leadership in support of integrated neighbourhood development across Livewell Southwest. Through policy interpretation, service redesign and partnership working, the role will help shape a practical strategic framework for neighbourhood-based care within the organisation and across system partners. Key areas of work will include: Provide GP-level advice on clinical governance, shared-care arrangements, escalation processes and risk management across Integrated Neighbourhood Teams. Support the safe design and implementation of multidisciplinary team models, proactive care, anticipatory care and integrated pathways across neighbourhood services. Build constructive relationships with GP practices, PCNs and wider partners so that neighbourhood models are grounded in frontline primary care insight and operational reality. Advise on evolving integrated care contracts and blended neighbourhood models, bringing clinical insight to questions of feasibility, accountability, governance and value. Represent the organisation in local system discussions where a primary care and neighbourhood perspective is required, helping align partners around practical and patient-centred models of care. Identify and support opportunities for innovation, including digital approaches and population health strategies that improve coordination, reduce inequalities and strengthen neighbourhood delivery. Clearly identify, declare and appropriately manage any potential conflicts of interest arising from concurrent roles within primary care or system partner organisations. Clinical Knowledge, Skills & Activities As a practising and experienced GP, the postholder will be expected to bring credible clinical insight, strong system awareness and the ability to apply general practice expertise to strategic service development. The GP Advisor should command professional credibility, demonstrate sound judgement and be able to translate policy and system change into practical neighbourhood approaches. To deliver the GP Advisor role effectively, you will need: A strong understanding of primary care, community services and the changing neighbourhood policy environment, with the ability to interpret national and local developments for organisational use. High-level interpersonal, political, analytical and relationship-building skills, with the confidence to influence senior leaders and system partners. A demonstrable understanding of governance, clinical safety, shared-care working and quality improvement in integrated models of care. Vision, influencing skills and practical judgement to support innovation, partnership working and the delivery of patient-centred neighbourhood care. Strategic Development, Planning & Organising The GP Advisor needs personal and professional credibility, with the ability to gain the confidence of executive leaders, operational colleagues, GPs and wider system partners. The role requires a positive, collaborative style and the ability to lead through influence, expert judgement and trusted relationships rather than formal management authority. You will need the ability to work across organisational and system boundaries to develop a shared vision for integrated neighbourhood care. This includes helping partners navigate complexity, balancing organisational and system priorities, and supporting governance arrangements that enable safe, clinically credible and sustainable change. The GP Advisor will be expected to: Make a significant contribution to the clinical leadership of neighbourhood transformation across Livewell Southwest. Support the development of neighbourhood models that are clinically robust, operationally feasible and aligned to organisational and system priorities. Advise on emerging neighbourhood policy, integrated provider frameworks and blended contractual arrangements relevant to the organisation. Provide clinical insight into risk, accountability and governance within integrated neighbourhood delivery models. Promote collaborative working between general practice, community services, mental health, social care and VCSE partners. Work with senior clinical and executive leaders to review progress, risks and opportunities linked to neighbourhood transformation. Support service improvement through clinically informed advice on pathway design, multidisciplinary working and integration with primary care. Contribute GP-led advice to discussions and negotiations relating to neighbourhood models, integrated care pathways and contractual development. Take an overview of how neighbourhood development can support population health, proactive care and improved outcomes across local communities. Work with internal and external partners to ensure effective governance and safe shared-care arrangements across neighbourhood pathways. Promote neighbourhood approaches that are inclusive, person-centred and responsive to health inequalities across the population. Encourage innovation and practical use of digital solutions where these strengthen care coordination, access and neighbourhood delivery. IT Systems & Processes The GP Advisor will support the use of digital and information systems that enable effective neighbourhood working, shared care and population health approaches. The role should help identify practical digital opportunities that improve coordination between services while remaining mindful of access, equity and the realities of frontline delivery. As part of this the GP Advisor will: Support the implementation of relevant organisational policies and digital approaches that enable integrated neighbourhood working. Advise on digital solutions that support multidisciplinary team coordination, shared care and improved communication across neighbourhood pathways. Support population health approaches by helping connect clinical insight, neighbourhood intelligence and service design.
