Summary

To demonstrate communication and motivational skills to facilitate patients to manage their health. The post holder will need to carry out the assessment, planning and delivery of interventions to a defined case load of patients. There may be complex and difficult issues to overcome to provide safe and effective care e.g. disengagement and refusal of service or high levels of risk to self and others supported by a senior clinician when appropriate. To communicate accurate and sensitive information to patients and carers using an appropriate and accessible medium, taking into consideration relevant factors such as background, culture, gender, age and any physical, sensory or learning difficulties. This communication may be complex or sensitive and needs to be modified as required to ensure effective understanding. To respect the dignity and preferences of patients and obtain consent to nursing intervention, in some circumstances you would be working in the patients best interests where consent cannot be gained due to capacity. Provide support and education to patients and carers and other relevant individuals considering patient confidentiality. Provide support and education to members of the multiprofessional team. To liaise with other team members, local services, and agencies to ensure the seamless transfer of patients within the pathway The post holder will be required to manage and prioritise their own time to ensure this efficiently meets the needs of the service, and when required to prioritise the work of other staff. This is all within a context of competing demands and fluctuating needs and risks. Ensure that discharge planning is commenced as soon after admission onto the nursing case load as is practicable and that patients/carers/family are empowered to make informed decisions affecting their future. The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group.
