Summary
SCOPE AND RANGE Conduct thorough clinical assessments of patients with neurological disorders to identify physical, cognitive, emotional, and functional needs. This includes assessing for impairments and barriers to rehabilitation. Develop individualised care plans tailored to each patient's unique needs and goals. Theseshould address specific neurological deficits, promote recovery, and facilitate the restoration of functional independence within the patients capabilities. Provide direct nursing care and interventions to address neurological symptoms, manage complications, administer medications, and perform treatments as devised and promoted by the multi-disciplinary team. Monitor patients' neurological status, vital signs, response to treatments, and progress in rehabilitation. Assess for any signs of deterioration or new symptoms that require immediate attention. Collaborate closely with the multidisciplinary team, including Doctors, Physiotherapists, Occupational Therapists, Speech and Language Therapists, Psychologists, Dietitians and other healthcare professionals, to ensure holistic and coordinated care for patients. Be an active participant in the inter-professional working of the MDT and input into the complete patient pathway, including acting as a Keyworker ensuring goals are met. Play a fundamental role in promoting patient safety, well-being, and independence, while also contributing to the ongoing development and improvement of neurological rehabilitation practices within the unit. Deliver evidence-based care, promote continuous learning, and professional growth to achieve optimal patient outcomes, enhancing the overall quality of care delivered in the neurological rehab unit. MAIN DUTIES/RESPONSIBILITIES To assess capacity, gain valid informed consent, document appropriately and demonstrate the ability to work within a legal framework with patients who lack capacity to consent to treatment. To comply with local confidentiality policies, child protection and vulnerable adult procedures. Support patient care through provision of clinical knowledge, clinical reasoning skills, practical clinical nursing skills and assist in teaching knowledge and clinical skills to other health care professionals. Provide education and support to patients and their families on the nature of their neurological condition, treatment options, self-care strategies, coping mechanisms, and community resources available to support their recovery. Maintain accurate and detailed documentation of assessments, care plans, interventions, progress notes, goal setting and outcomes to support continuity of care and adhering to LiveWell Southwest policies and procedures Participate in clinical audits and quality improvement initiatives to enhance the standard of care and promote evidence-based practice. Advocate for patients' needs, rights, and preferences within the healthcare system to ensure they receive high-quality and person-centred care that promotes their well-being and recovery. Participate in the education and assessment of performance of students and junior staff, in order to provide competent advice and clinical support whilst promoting the effective and efficient delivery of a quality service. Responsibility for People Management Provide guidance and education to less experienced staff, including health care assistants and student nurses, ensuring they understand their role and responsibility with the neurological rehab unit. This will include the use of formal appraisal, line management, practice supervision and use of related documentation. Participate in induction and support programmes in relation to new staff and volunteers. Take responsibility for own personal and professional development, maintain competence, knowledge and skills commensurate with the role. Coordinate and delegate tasks effectively to optimise patient care. Be an active member of the in-service training program by the attendance and delivery of presentations and training sessions at staff meetings, tutorials, in-house training sessions and by attending external courses and practicing reflective practice to develop own and others skills, knowledge and expertise. To assist in the recruitment of relevant grades of staff, as required. Take a flexible approach to supporting colleagues in times of caseload pressure, including working with colleagues to maintain clinical cover across the team within the resources available. Foster a collaborative and supportive team environment by promoting a culture of open communication, mutual respect and effective teamwork amongst all staff members. Responsibility for financial and/or physical resources The post holder will have no direct responsibility for finances; however, they will adhere to the relevant trust policies on financial issues. The post holder will use Livewell Southwest resources responsibly ensuring that resources are utilised effectively to meet changing service needs. Responsibility for administration To ensure competency in record keeping, maintaining an up-to-date record of all patient contacts, including MDT care plans, goals, assessments, observation charts and reports whilst always ensuring confidentiality in line with professional and organisational guidelines. To report accidents, incidents or complaints following LSW procedures and policies and to comply with the most recent Health and Safety at Work Act. To attend and contribute to relevant team/service meetings. To access and respond to work related emails and other information on a regular basis through Livewell Southwest intranet Responsibility for people who use our services Be an autonomous practitioner, working within appropriate professional guidelines and code of conduct. To undertake the comprehensive assessment of neurological inpatients, including those with a complex presentation, using investigative and analytical skills, to formulate individualised management, treatment plans and patient goals in collaboration with the MDT, using clinical reasoning and utilising a wide range of treatment skills and options. To ensure a high standard of clinical care and support less experienced staff to do likewise. Act as a patient advocate ensuring individual needs, preferences and choices are delivered by the service. Provide specialist advice via written or verbal instruction to patients, carers and relatives to promote understanding of the aims of rehabilitation, and to ensure a consistent approach to patient care. This may be health education, preventative or condition- specific advice. Evaluate patient progress, reassess and liaise with MDT to discuss changes. To have a flexible approach to competing priorities to meet the needs of the patient and the service. To work jointly with other health and social care colleagues and services (for example GPs, District Nursing, Social Services, private sector and therapy colleagues) to ensure patients identified rehabilitation and care needs are met through delivery of a coordinated multidisciplinary service. This will include goal setting meetings, progress meetings and discharge planning. To comply with local confidentiality policies and be able to respond effectively to any concerns of abuse/safeguarding and know procedures they need to follow to escalate their concerns. Comply with all infection control policies and guidance and attend relevant updates to support the needs of the service. To ensure privacy, dignity and human rights are respected for all patients and carers. Responsibility for implementation of policy and/or service developments Ensure adherence to Livewell Southwest Policies. To demonstrate a sound understanding of Clinical Governance, Risk Management and apply to work situation. Demonstrate an understanding of clinical practice developments, utilising research in therapeutic interventions to develop and deliver evidence-based care. To maintain own clinical professional development (CPD) by keeping abreast of any new trends and developments, National Service Frameworks and NICE guidelines, and incorporate them as necessary into your own practice through participation in effective use of professional and operational supervision and appraisal. To participate in the staff appraisal scheme and Personal Development Plan (PDP) as both appraiser and appraisee. The post holder will be aware of and work within their own limits, and competencies.
