Summary

Role Overview We are seeking a Clinical Pharmacist to join Awburn House Medical Practice in a practice-based role, supporting day-to-day patient care, medicines optimisation, and reduction of GP workload. The role focuses on patient-facing consultations, structured medication reviews, and operational prescribing support within the practice, rather than PCN-level leadership or system-wide service development. Patient Care & Medication Reviews Undertake structured medication reviews for patients with complex needs, polypharmacy or high-risk medicines Support patients with long-term conditions to optimise treatment and improve outcomes Identify and resolve medication-related issues, including adherence and safety concerns Deliver patient-centred consultations, supporting shared decision-making Promote patient understanding and self-management Prescribing & Medicines Management Prescribe within scope of competence (Independent Prescriber desirable, not essential) Support safe, day-to-day prescribing within the practice setting in line with NICE, MHRA and local guidance Complete medicines reconciliation following hospital discharge to support continuity of care Monitor and manage patients on high-risk medications Practice-Based Clinical Support Provide day-to-day medicines-related advice to clinicians and staff within the practice Support and improve repeat prescribing systems and workflows at practice level Manage medication queries from patients and the practice team Contribute to reducing GP workload through clinical pharmacy support Quality Improvement & Governance Support safe and cost-effective prescribing within the practice Participate in clinical audits and quality improvement activity Implement safety alerts and prescribing updates as required Help ensure compliance with CQC standards and medicines governance requirements Team Working & Collaboration Work as part of the Awburn House multidisciplinary team Liaise with community pharmacies, hospitals and other healthcare providers as required to support individual patient care Contribute to practice-based clinical meetings and patient case discussions This role does not include PCN-level leadership or system-wide service development responsibilities Education & Development Provide medicines-related advice and education to patients and staff Support team learning and development in prescribing and medicines use Maintain ongoing professional development and clinical knowledge
