Summary


This is an exciting opportunity to take a lead role in contributing to the delivery of high-quality care within the South Manchester Home-Based Treatment Team. As a Band 7 Senior Practitioner, you will provide strong leadership and support the management of a busy urgent care team to ensure the delivery of safe, effective, and compassionate care for individuals requiring HBTT as an alternative to hospital admission.

The Senior Practitioner will support the day-to-day coordination and management of the service. You will provide visible and accessible leadership to staff within the Service; you will support the delivery of a safe and efficient service to those accessing it and ensure timely, person-centred interventions that promote recovery and wellbeing. Key Aspects of the role include monitoring the performance and quality of the service the team delivers. Alongside clinical leadership and effective day-to-day management of the team.



You will need to be able to manage your own workload, work well under pressure and be able to work within a multi-disciplinary team. You will also be working collaboratively with multiple agencies and will need to demonstrate excellent interpersonal skills and an awareness of Trust Policies and Procedures.



The HBTT service operates 24 hours a day, 7 days a week, so flexibility in working patterns is essential, which may include weekend and out-of-hours working and contributing to the on-call Operational rota.




Supporting the service to develop and maintain robust gatekeeping processes, incorporating the principles of person-centred care and least restrictive interventions

Proactively identifying and facilitating supported early discharges from our inpatient settings, by attending board rounds and mini-MDTs, and completing trusted assessment for patients going to out of area HBTs on discharge

Being involved in purposeful admission formulation and statements, and the review of these as part of ward round if and when admitted and with other services involved such as community mental health teams
• SURP provides training on HBT and the SURP role to key stakeholders, for examples inpatient services and MHLS, and to promote this role and understanding of the role of HBT.

They play an active part in the leadership and service development of the HBT, for example by line managing staff members, offering clinical supervision to staff either formally or informally, attending and contributing to the leadership meetings and supporting operational duties.

- SURP facilitate spoke days for shadowing as part of induction or on student placements – this can be from HBT or other services across the division.

Ø Undertake responsibility for the continuous improvement of the service provided by the Home-Based Treatment Team.



Ø Ensure that the contract for the service is delivered in line with the service level agreements and service specifications which detail quality, quantity and cost. Including the provision of regular reports on the agreed service standards, performance indicators and outcomes.



Ø Undertake responsibility for the maintenance of good standards of professional practice.



Ø Undertake responsibility for the management of the financial budgets for the Home-Based Treatment Team in accordance with the Trusts standing financial instructions.



Ø Undertake line management responsibility for all non-medical staff in the Home-Based Treatment Team.



Ø To lead the multi-disciplinary team



Ø To plan with the multi-disciplinary team the allocation of key workers and involvement of other team members in treatment plans.



Ø To undertake risk assessment and risk management of all clients as necessary.
