Summary
Main Duties and Responsibilities 1 Work in partnership with the team manager to maintain a continued responsibility for an identified caseload. 2 To assist in identifying children and young people with a working diagnosis of asthma orwheeze that would benefit from one to one intervention to support self-management of theirasthma 3 To ensure clinically effective practice of specialist respiratory treatment through assessment, planning, implementation and evaluation of work carried out in a timely manner. 4 To be proactive in reducing acute asthma activity, admissions, accident and emergency attendances and outpatient activity. Ensure children are followed up appropriately and in collaboration with staff within the asthma service. 5 Provide ongoing support and education to children, young people and families within the home, school or clinic setting. To ensure all relevant information is available to enable them to make an informed choice about their treatment and care. 6 Be temporarily responsible for the caseload of other team members during absence, annual leave and acting as a resource for colleagues as required. 7 Act as shift coordinator for team on a rotational basis. This involves ensuring personal safety of other team members/ signing off as per trust policy, liaison with Rainbow ward, SCBU, Paediatric A & E regarding referrals, checking telephone messages and responding as appropriate. 8 Assess and order equipment and supplies for the child or young person to enable them to manage their condition safely in the community setting, including regular reassessments dependent on the changing needs of the child/young person. 9 Provide high quality nursing care for children and young people in the London borough of Newham which is planned and delivered, reviewed and evaluated within the clients home, asthma clinic or other appropriate setting within the community using family centered, evidence based practice. 10 Provide initial triage and assessment of new referrals to the service, identifying where urgent response is required and allocating to staff, based on staff logistics and skill mix. 11 Initiate and manage safe, effective discharge from hospital to home carrying out risk assessments of the receiving community setting. 12 Attend, and where appropriate, lead in discharge planning meetings. 13 Maintain a transparent diary of work and record journeys made in the required manner. 14 Provide effective clinical care in less than conducive environments. 15 Enable and promote independence by teaching, supporting and encouraging carers and children, including those children with complex health needs, to participate in meeting their own care needs where possible in community settings. 16 Initiate and manage appropriate changes in care in negotiation with children, families and professionals following review of planned care either at case review, morning handover or joint visits. 17 Establish and maintain open and effective channels of communication with members of Community Childrens Nursing Services, the wider MDT and staff from other settings. 18 Work confidently as a lone worker adhering to Trust Policy and procedures at all times. 19 Provide emotional support to clients and families who may be facing difficult decisions, bad news or bereavement. 20 Provide seamless clinical care and emotional support for families from a diverse multi-cultural, multi-ethnic and economically deprived areas respecting through heightened awareness their cultural, religious and social beliefs. 21 Communicate with families for whom English is not their first language and/or children with communication difficulties/ special needs. This involves working in collaboration with health advocates (interpreters), or using highly developed verbal/ non-verbal communication skills. 22 Be responsible for management and reporting of incidents, comments and complaints from patients, carers or relatives and junior staff, in accordance with the Trust policies and procedures, and complete all appropriate paperwork. 23 Initiate and participate in patients reviews and case conferences with other Professionals and provide reports using computer software as required. E.g.: Social Services, health professionals, education. 24 Attend weekly/monthly team meetings, management meetings, daily handovers and other briefings for Newham Community Childrens Nursing Service and in other professional setting as required 25 Provide flexible visits to children and young people as required (before and after school). 26 Work in collaboration with specific consultants to manage specialist out patients clinics to enable clients to receive multidisciplinary assessment e.g. asthma, enteral and oxygen dependent clinics. 27 Collect and collate information for audit/ research purposes, the results of which will promote evidence based childrens community nursing service. E.g.: new oncology protocols/ trials of treatment and oncology research.
