Crynodeb
The post-holder will contribute 4 - 6 clinical sessions per week at Ellison View Surgery, including home visits as required. Responsibilities will be in line with the agreed practice timetable and will include a combination of: Conducting surgery consultations, telephone consultations, and managing patient queries. Visiting patients at home as clinically necessary. Reviewing, authorising, and managing repeat prescriptions and associated queries. Completing paperwork and correspondence in a timely and efficient manner. The post-holder will be expected to: Make professional, autonomous clinical decisions regarding presenting problems, whether self-referred or referred by other healthcare professionals within the practice. Assess and manage the healthcare needs of patients with undifferentiated or undiagnosed conditions. Screen patients for disease risk factors and early signs of illness. Develop patient-centred care plans in consultation with patients, in line with current disease management protocols. Provide counselling, health education, and support for self-management. Admit, discharge, or refer patients to other care providers as appropriate. Record clear, contemporaneous consultation notes in accordance with agreed standards. Collect data for audit and quality improvement purposes. Prescribe in line with the practice formulary, generating computerised prescriptions wherever possible. In addition to clinical duties, the post-holder will: Comply with all relevant practice policies, including prescribing, confidentiality, data protection, and health and safety. Commit to ongoing professional development, audit, and evidence-based practice. Contribute to evaluation, audit, and clinical standard-setting within the practice. Support the development and optimisation of computer-based patient records. Attend training, workshops, and practice-organised events as appropriate. Overall, the post-holder will perform all duties expected of a GP practising within a primary care setting, delivering safe, effective, and patient-centred care
