Crynodeb

Conduct Structured Medication Reviews (SMRs) and postdischarge medicines reconciliation. Manage repeat prescribing, eRD, formulary adherence, and costeffective prescribing. Run clinical sessions (e.g., hypertension treatment escalation, asthma/COPD optimization, polypharmacy, deprescribing). Support QOF, IIF/PCN objectives and quality improvement projects/audits. Provide clinical advice to GPs, nurses, HCAs and care coordinators; contribute to MDT huddles/meetings. Handle medication queries from patients and care homes; deliver patient education. Implement prescribing safety measures (e.g., highrisk drugs monitoring, MHRA alerts). Develop and update practice protocols/SOPs; contribute to clinical governance. (If IP qualified) Provide independent prescribing clinics within scope of competence
