Summary
Background We are looking for a dynamic and experienced team member to provide administrative support to our Primary Care Network (PCN 7&8) Teams in the East of Tower Hamlets. You will be employed by the Poplar & Limehouse Health and Wellbeing Network Overview of your organisation Poplar & Limehouse Health Network CIC (Primary Care Network PCN) - Who we are We are a partnership of local general practices, community care teams, and local third-sector providers Our ambition is to continue to pioneer the development of a dynamic partnership of health and social care services across the Poplar & Limehouse. We will build on the strength of current local NHS general practice, social care and 3rd sector provision to: access and manage resources effectively; initiate new and innovative models of service delivery to address health inequalities across the patch and improve patient and public experience of the services Weendeavour topromote independence, choice and control by users of our services and provide seamless integrated care to individuals Our Aim is to keep quality high, services equitable and bureaucracy low in a partnership of mutual trust, respect and vision. We will continue to deliver high quality health care whilst instigating/engaging-in initiatives to tackle the wider determinants of ill-health We work together in a collaborative model, to maximize the benefits to our patients and community while preserving the integrity of each individual Practice For this post, we are working with one other like-minded PCN (PCN8), the Healthy Island Partnership The Role You will support the professional operation of both PCNs through various clerical, organisational and communication tasks You will need to be both effective and efficient to be able to undertake and prioritise a variety of administrative duties You will occupy a position of responsibility and trust, as you have access to sensitive information.
