To support Technicians and Pharmacistsin Acute Medical Admissions at Tameside and Glossop Integrated Care NHS Foundation Trust (T&G ICNHSFT),to provide Medicines Management services. On occasion to provide in reach into our Emergency Department and Same Day Emergency Care-SDEC. The post holder will work under the supervision of a Pharmacy Technician toassistin the delivery of pharmaceutical care to patients within the Trust through the provision of a safe, efficient andcost effectivemedicines management programme.
The role will involve direct patient contact.
• Optimises patients own medicines brought into hospital through direct or remote discussion with patients/family relatives/carers
• Uses interpersonal skills to communicate the medicines optimisation strategy to the multidisciplinary team
• Uses interpersonal skills toensure patients/relatives/carers understand the advantages of the Trusts Medicines Management Programme. This will require the use of excellent communication, good persuasiveskillsand knowledge of how to overcome barriers to understanding
• Gains consent for the patients Summary care Record to be viewed, ensuring full compliance with access regulations and patient confidentiality procedures
• Records information relating to thepatientsmedication onto the prescriptionsheet or in the future the electronicsystem
• Followingappropriate trainingand oncedeemedcompetent to do so,participatein themedicinesreconciliation service at ward level. This will involve ordering medication, assessing patients ownmedicinesand assessing the requirements for compliance aids.
