We are looking for a compassionate and motivated Support Worker to join our dynamic Community Mental Health Team.You’llbe working alongside nurses, social workers, psychologists, and psychiatrists to support adults living with mental health conditions in their recovery journey.As a Community Mental Health Support Worker, you will play a vital role in supporting individuals experiencing mental health challenges within their local communities. Working as part of a multidisciplinary team, you will help service users achieve their recovery goals,maintainindependence, and improve their overall wellbeing.
• Provide practical and emotional support to service users in their homes and community settings.

• Assistservice users to develop theirdaily living skills, including budgeting, cooking, and accessing local services.


• Support care planning and recovery-focused interventionsalongside other members of the multidisciplinary team.

• Encourage engagement in social, educational, and vocational activities.

• Monitorservice user mental state and escalate concerns asappropriate.

• Promote independence, dignity, and choice in all aspects of care.

• Work within a multidisciplinary community mental health team, contributing to team discussions.

You will work collaboratively with nurses, social workers, psychologists, psychiatrists, and other professionals to deliver person-centred care within community settings.

The ideal candidate will have a genuine passion for mental health, excellent communication skills, and the ability to build trusting relationships with service users. You will empower individuals to develop practical life skills, engage in meaningful activities, and access community resources that promote recovery and resilience.

Key responsibilities:
• Direct Support & Engagement
• Provide practical and emotional support to service users in their homes and community environments.
• Assist individuals in developing daily living skills such as budgeting, meal preparation, and household management.
• Encourage participation in social, educational, and vocational activities to reduce isolation and promote inclusion.
• Recovery Planning & Monitoring
• Contribute to care planning and implement recovery-focused interventions in collaboration with the multidisciplinary team.
• Monitor service users’ mental health and wellbeing, promptly escalating concerns to clinical staff.
• Support service users in setting and achieving personal recovery goals.
• Advocacy & Empowerment
• Promote independence, dignity, and choice in all aspects of care.
• Assist service users in accessing local services, benefits, and community resources.
• Advocate for service users’ needs and preferences within the team and wider community.
• Teamwork & Communication
• Work effectively within a multidisciplinary team, attending meetings and contributing to case discussions.
• Maintain accurate and timely records in line with organisational policies and confidentiality standards.
• Communicate professionally with service users, families, and external agencies.
