Summary
We are seeking a compassionate, proactive, and motivated Community Mental Health Support Worker to join our dynamic multidisciplinary team. This role is pivotal in supporting adults living with mental health conditions to achieve their recovery goals, maintain independence, and enhance their overall wellbeing. You will work collaboratively with nurses, social workers, psychologists, psychiatrists, and other professionals to deliver person-centred care within community settings. The ideal candidate will have a genuine passion for mental health, excellent communication skills, and the ability to build trusting relationships with service users. You will empower individuals to develop practical life skills, engage in meaningful activities, and access community resources that promote recovery and resilience. Key responsibilities: Direct Support & Engagement Provide practical and emotional support to service users in their homes and community environments. Assist individuals in developing daily living skills such as budgeting, meal preparation, and household management. Encourage participation in social, educational, and vocational activities to reduce isolation and promote inclusion. Recovery Planning & Monitoring Contribute to care planning and implement recovery-focused interventions in collaboration with the multidisciplinary team. Monitor service users mental health and wellbeing, promptly escalating concerns to clinical staff. Support service users in setting and achieving personal recovery goals. Advocacy & Empowerment Promote independence, dignity, and choice in all aspects of care. Assist service users in accessing local services, benefits, and community resources. Advocate for service users needs and preferences within the team and wider community. Teamwork & Communication Work effectively within a multidisciplinary team, attending meetings and contributing to case discussions. Maintain accurate and timely records in line with organisational policies and confidentiality standards. Communicate professionally with service users, families, and external agencies. Compliance & Professional Standards Adhere to safeguarding protocols and health and safety regulations. Uphold organisational values and equality, diversity, and inclusion principles. Participate in training, supervision, and continuous professional development activities.
