Summary
Duties Manage a patient caseload with chronic diseases, including diabetes, asthma, COPD, hypertension, and cardiovascular conditions. Develop, implement, and evaluate personalised care plans in partnership with patients, families, and carers. Deliver disease specific clinics and opportunistic screening, including blood pressure, BMI, blood glucose, ECGs, and spirometry as required Deliver comprehensive patient care, including administering medication, monitoring vital signs, and assisting with daily activities. Support doctors and other healthcare professionals during examinations and procedures. Maintain accurate and detailed patient records in accordance with legal and organisational standards. Educate patients and their families on health management, treatment plans, and post-discharge care. Assist in the organisation of patient flow to optimise efficiency within the healthcare facility. Ensure a clean, safe, and welcoming environment for patients and staff by adhering to health and safety protocols. Respond promptly to patient needs and emergencies with professionalism and composure
