POSITION: GP Health & Wellbeing Worker Role
LOCATION: LIVERPOOL
HOURS: 40 HOURS PER WEEKSALARY: £26,208.00
SUMMARY DESCRIPTION
As a member of Liverpool Carers Centre, to identify unpaid carers and support them to access practical and emotional support. You will aid in empowering carers to take control of their own health and wellbeing. You will give time and focus on the what matters to me philosophy and take a holistic approach to connect carers to community groups and statutory services.
Core Responsibilities:
SCOPE OF ROLE:
- You will be an energetic and solution focused worker who will support carers to access the most
appropriate services to support them in their caring role.
- Working throughout the City you will develop relationships that help and support unpaid carers while being a main enabler for holistic solutions, Car owner/driver is essential.
ACCOUNTABLE FOR:
- Developing and sustainment of Health & Wellbeing Support
- Supporting collaborative working
- To provide advice, guidance, and practical support in the implementation of support packages
- Being one of the key contacts on for the Health & Wellbeing project for Local Solutions, particular around GP Surgeries and Health Care.
RESPONSIBILITIES:
- To work with GP practices and Health Care settings to raise awareness and identify unpaid
carers and carers known to them.
- To promote the pathway of receiving referrals from GP surgeries re carers.
- To support carers to access the relevant support dependant on their need.
- To give presentations, attend meetings and provided information to GP practice staff and other
organisations.
- To be part of the health and wellbeing team within the carers centre and take part.
in events and activities that will offer support to carers.
- To complete administrative tasks using excel and other soft ware systems.
- To use a variety of social media platforms to help raise awareness.
- To provide monthly KPIs and reports as required.
- To take on other roles that will support this piece of work.
ACCOUNTABILITIES:
- You will be accountable for working and supporting GPs and Health Care to raise awareness recognition of unpaid carers and put in place support to meet carers needs.
- You will work to targets on the number of referrals received as a result of the support provided.
- Ensuring new referral information is added accurately to databases and systems
- Adhere to data protection legislation and data sharing agreements, ensure that all carers that are supported by the service are accurately recorded using agreed templates, following up to ensure that they have received good support; completing monitoring as required, to demonstrate outcomes of the service.
ADDITIONAL DUTIES:
- To perform any other duties appropriate to this post, as necessary or as requested
- Participate in professional development, using appropriate learning opportunities, to improve relevant knowledge and skills.
- Ensure that all activities of the project provide equality of opportunity
GENERAL SKILLS AND EXPERIENCE:
- Experience of working in health or social care.
- Experience of supporting unpaid carers or collaborating with people.
- Experience of working as part of a team but the ability to prioritise own workload, managing own
diary, building relationships with both carers and professionals, databases, taking referrals from
and inputting information.
- Attendance at occasional evening or weekend events.
- Technical expertise should include ability to use Microsoft office, Excel, Outlook
- The successful candidate will require an enhanced Disclosure & Barring Check and undertake
mandatory safeguarding training as appropriate for the role.
- Travel is required so a car owner/driver is essential as this is a city-wide post
