Summary
We are looking for someone to support, encourage, and motivate patients in improving their mental health and overall wellbeing through empowering, person-centred interventions. The successful post holder will provide personalised, holistic support to patients and will: Coach and motivate patients through multiple sessions to help them identify their strengths, set meaningful goals, and take active steps towards increased social engagement and community involvement. Deliver interventions such as self-management education, peer support, and group sessions for patients and families where appropriate. Facilitate access to support services, including financial and benefit advice, housing, health and safety home systems, and leisure/social activities. This may involve accompanying patients to appointments and groups. Make referrals to local community groups and other agencies, sharing information and identifying gaps in provision. Assist patients in building resilience and making informed decisions about their health and wellbeing. Work in partnership with GPs, Mental Health Practitioners, and the wider multidisciplinary team to connect patients to community-based activities and services that enable them to take increased control of their health and wellbeing. Develop networks and partnerships with other agencies, attend multi-disciplinary meetings, and participate in local events to raise awareness of the service. Utilise health coaching skills to help patients develop the knowledge, skills, and confidence to manage their health and wellbeing, while increasing their ability to access and use community support offers.
