An exciting opportunity has arisen for Band 5 Community Nurse working within the Woodbridge Integrated Neighbourhood Team. We are a busy dynamic team who actively support staff new to the community. The position being offered is on a permanent basis at 22.5 hours per week. Experience is preferred but not essential as full training will be given.
The motivated and forward-thinking team consists of Nurses, Occupational therapists, Physiotherapists, Social Workers and Support Staff. The team also work closely with other internal and external services or agencies.
The primary function of the Integrated Neighbourhood Team (INT) is to provide healthcare in the patient/customers home environment. You will be part of a multidisciplinary team providing interventions, such as short-term rehabilitation, wound care, and end of life care. The INT also supports with admission avoidance, timely discharge, and promotion of independence through holistic assessment and informed choice. In addition to the visiting patients/customers within their own environment, the team also have the use of a gym and clinic setting at Stow Lodge Centre. These spaces are used to run clinics, such as Falls Clinic, Better Balance group, Leg Ulcer clinic, catheter clinic, PICC Line Care.
The team work closely with 7 GP practices and there is a wealth of experience and knowledge within the current members of the team.

An exciting rotational opportunity has arisen within Woodbridge’s well established leg ulcer clinic with the opportunity to rotate into the community nursing teams, as well as to support with the delivery of our other non-housebound clinic which manages other care needs, such as catheter care, PICC line management, rocket drain management and VAC therapy.
Leg ulcers are of huge socio-economic importance costing the NHS over one billion pounds per year. There are in excess of 100,000 active venous ulcers in the UK at any one time, 80% of these have treatment that is based in the community.
We are looking for a permanent band 5 registered nurse to be contracted for 22.5 Hours a week.
The Woodbridge leg ulcer clinic runs a busy clinic treating all ambulant leg ulcer patients across Woodbridge. It operates 1 clinic room currently located at our Woodbridge Integrated Clinic at Riduna Park in Melton, 3 days a week and is very successful with meeting government healing targets. It is run by a small well knowledgeable specialist team made up of a total of 2 trained staff.
Woodbridge has a large community nursing service offering holistic and sometimes complex care in people’s homes. You will not only be working alongside other Community and District Nurses but also with a larger team of phlebotomists, assistant practitioners, skilled Generic Support Workers, Physiotherapist, Occupational Therapists and team administration colleagues.
