Offer appropriate care to patients of all ages and clinical needs. Assess, diagnose, investigate, treat, and refer patients as required, ensuring timely and safe clinical decision-making. Participate in on-call duties and other scheduled clinical sessions as required. Support and promote end-of-life care frameworks (e.g., Gold Standards Framework) to ensure high-quality palliative care. Manage long-term conditions in line with national guidelines, pathways, and best practice. Prescribe medications appropriately, using generic prescribing where suitable, and provide evidence-based rationale when alternatives are required. Provide health promotion, prevention advice, and lifestyle counselling to support patient wellbeing. Non-Clinical Responsibilities Complete administrative and operational tasks required to fulfil GMS contract obligations, including referrals and documentation. Maintain accurate, up-to-date clinical records in accordance with professional standards, using appropriate coding systems (e.g., READ, SNOMED). Work collaboratively with IT teams and data analysts to generate performance and service-monitoring reports. Liaise with external agencies, services, and community organisations to support patient care pathways and raise awareness of available services. Work with colleagues to support demand management and improve service efficiency. Support GP leads and management in delivering service improvement plans, including Practice-Based Commissioning (PBC) developments. Promote the use of key NHS initiatives (e.g., Choose and Book, cancer pathways, 18-week referral targets). Encourage and support patient participation in service improvement and feedback processes. Comply with all relevant NHS policies, including safeguarding, HR processes, and clinical governance. Participate actively in practice meetings and wider team forums to support communication, service development, and quality improvement. Training and Education Provide health education and self-management advice to patients and carers. Participate in the annual appraisal process. Undertake mandatory and developmental training to maintain clinical competencies and support service needs. Contribute to practice development projects as agreed with the management team. Maintain up-to-date Continuing Professional Development (CPD) records. Share learning with colleagues through teaching, case discussions, significant event analysis, and clinical review meetings. Clinical Governance, Research and Audit Participate in clinical audits to monitor quality and identify opportunities for improvement. Engage in research, audit activity, and evidence-based practice where appropriate. Assist in the development and implementation of practice policies, protocols, and clinical guidelines. Ensure services are informed by current evidence and aligned with the clinical needs of the practice population. Communication Maintain excellent communication with patients and carers to ensure informed decision-making and continuity of care. Promote effective communication within the multidisciplinary team to support good clinical and operational practice. Build and maintain constructive relationships with secondary care services, external partners, and community providers.
