Summary

Advanced Clinical Practitioner Extensive care / Urgent Crisis response
Band 8a

Two posts - one 30 Hour post + one 36 hour post

Urgent Crisis response & Extensive care is an Advanced Clinical Practitioner led service based in the community. The team provide urgent health care to patients in their homes who are experiencing an acute illness, to avoid an unnecessary admission to hospital.

Extensive care is the management and optimisation of patients with Long term conditions.

Working within the team you will be part of developing and shaping the future of the two - hour response service within Tameside and Glossop along with the developments within Extensive care to move towards a community frailty model.

The post holder will be an autonomous Advanced clinical practitioner who will be expected to take an active role in the response, stabilisation and making safe element of acute care pathways by delivery of advanced assessment, management, and treatment of patients over 18 years of age within the Extensive Care & urgent crisis response service.

The successful postholder will have community experience and demonstrate specialist clinical skills and knowledge in advanced clinical assessment. The Advanced clinical practitioner will offer clinical expertise, guidance, and management plans,

1. To act as a role model of expert clinical practice.

2. To ensure that documentation is maintained to a high standard. Be a competent user of and participate in the ongoing development of the electronic patient record.

3. Ensure that patients receive high quality clinical care including medical diagnosis.

4. Ensure that effective discharge planning and health promotion is carried out.

5. To monitor the quality and continuity of patient care and ensure standards are met.

6. Establish and maintain effective communication with nursing, medical and paramedical colleagues to provide optimum patient care.

7. Communicate effectively with patients, relatives, and colleagues.

8. Participate in audit, taking action to address improvements required

9. Practice in such a way that inspires confidence in all people using the service

10. Provide advanced levels of practice, knowledge and skills within the Urgent Care Pathway.

11. Develop and implement care pathways for the management of groups of patients within Urgent Care.

12. Promote evidence based practice through the use of the latest research based guidelines and the development of practice based research.

13. Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review
