The opportunity has arisen for a band 7 Diabetes Nurse to join the Community Diabetes service to provide care to people registered with a GP in Bexley.
We are seeking a motivated, enthusiastic individual who is committed to developing and improving diabetes services, ensuring patients receive a high-quality service in the Bexley locality. Within this role the post holder will holistically assess and agree a personalised plan of care for people with Diabetes. The services will be delivered in a variety of community locations including outpatient clinics and travelling in the borough independently is expected.
Applicants must be able to work on their own as well as being part of a multi-disciplinary team (MDT). You will be involved in training and educating staff and members of the MDT. You will need to be an effective team player and committed to providing a high standard of care. The post holder will also be supported by the Diabetes Team lead.
Excellent communication skills are required in order to empower, support and educate patients as well as community and practice staff to improve the quality of diabetes care with a focus on type 2 diabetes. The post holder will use their clinical experience and knowledge to contribute to both guideline and service development in this area. Our Team is committed to evidence-based practice and service improvement.
The post holder will work within the diabetes team to provide specialised expertise in practice, facilitation, and management of change through collaboration, providing vision, education, research and practice development.
They will offer patients, relatives and carers support and advice by providing education tailored to suit individual needs and will provide specialist expertise and education in diabetes care to GPs, practice nurses, patients, relatives, carers and other health care professionals and will support junior staff in doing so.
The post holder will provide line management to the Bexley Band 6 Diabetes Specialist Nurses within the Community Diabetes Service. The post holder will work autonomously, prioritise own workload and undertake and develop nurse led clinics within the community.

• Use advanced clinical skills for assessing and treating patients with diabetes using relevant clinical protocols for newly diagnosed patients with diabetes, patients with unstable diabetes and patients with complex needs and/ or complications of diabetes
• To be professionally and legally responsible and accountable for all professional activities.
• To be able to work alone in the community in a variety of settings, independent of multidisciplinary support whilst in direct contact with patients and their carers.
• To provide line management to Band 6 Diabetes specialist nurses within the Community Diabetes Team, including supervision, performance management and appraisal.
• To contribute to the planning and development of the diabetes service in Bexley in accordance with National guidance.
• To participate in the continued development and implementation of nurse led clinics in the community for diabetes patients.
• To work in a co-ordinated manner, communicating effectively with other team members and the wider clinical network involved in patient care.
• To maintain up-to-date clinical written records and electronic database in accordance with local and professional standards.
• To be able to identify when and how to terminate involvement or refer onwards to appropriate teams/services and complete input in a timely, seamless manner.
• To seek guidance from more experienced colleagues when required.
• Capability to deputise for senior colleagues where appropriate.
