Summary
West London NHS Trust is seeking a skilled and motivatedNurse or Physiotherapistto join theHounslow Community Respiratory Service. This Band 6 role offers an exciting opportunity to deliver specialist, patient-centred rehabilitation for adults with long-termcardiac and respiratory conditions, including those diagnosed with Long COVID, acrosshome, clinic, community venues, and virtual platforms.
The successful candidate will provideholistic assessment, rehabilitation, and case managementto support patients in maximising independence, functional ability, and quality of life. You will work collaboratively within amultidisciplinary teamto support Pulmonary and Cardiac Rehabilitation (PR/CR), Early Supported Discharge (ESD), chest physiotherapy, and specialist interventions delivered through HOSAR.
· DeliveringPulmonary and Cardiac Rehabilitation programmesin line with NICE, BTS, and ACPICR guidance.
· Providingspecialist assessment and case managementfor adults with long-term cardiac or respiratory conditions.
· Supporting patients withbreathlessness, fatigue, anxiety, and functional limitations.
· Deliveringchest physiotherapy and oxygen therapy interventionswithin the community.
