Are you passionate about improving the mental and physical wellbeing of care home residents? We’re seeking a Senior Mental Health Practitionerto join ourCare Home Support Team, working alongside Registered Nurses, Therapists, GPs and wider MDT colleagues to deliver holistic, person-centred care across East Berkshire.
This is a unique role for someone who is passionate about supporting care homes to build their knowledge and embed therapeutic techniques through teaching, reviewing care plans, signposting and guiding care pathways. You’ll have the opportunity to support holistic management of complex mental health needs, acting as a key resource for care home teams to enabling them to ensure residents receive safe, person-centred care to the highest standards in line with the Enhanced Health in Care Homes Framework.
You will work closely with care home staff and multidisciplinary teams, using coaching techniques to build staff confidence and competence in managing mental health conditions, promoting best practice and reducing avoidable hospital admissions. Giving care homes the opportunities engage in areas of practice that they wish to develop and enabling you to embark in projects to improve mental health care.
Excellent communication, compassion and a holistic approach are key in this role in driving improvements in resident care.

• To build good working relationships withthe carehomes in the area.
• Advisecarehomes on management ofdementia, mental health symptoms and management, mental capacity, DOLs and safeguarding.
• Supportimplementing service redesign initiatives which meet local and national priorities and are in line with commissioning intentions.
• Provide education to care homes via TEAMS or face toface onmental health issues, championworkand areas of care the home has identified as a need. This could be formal or informally with rolemodelling/upskilling.
• Provide a holistic assessment to aid medical diagnosis,managementand treatment of complex residents in the care home environment and prevent unnecessary hospital admissions or placement failure.
• Work in collaboration with other members of the MDT, Mental Health teams,LACare QualityteamsandtheIntegrated Care Board.
