Summary
To promote the philosophy of person-centred care, establish and maintain a supportive relationship with people affected by cancer. To work as an autonomous practitioner and has the freedom to act to influence patient care and service provision. To work within the specialist cancer site-specific Multi-disciplinary Team (MDT) within the Betsi Cadwaladr University Health Board, be a core member of the MDT and the designated named lead nurse within the team for those patients. To be responsible for the management, delivery and on-going future development of a high quality, accessible nurse led service. To put quality at the heart of practice by delivering evidence based, individualised and personalised care through holistic needs assessment, planning and evaluation of care and all care interventions.
