Summary

Were looking for a dynamic and skilled practitioner to join our Continuing Healthcare (CHC) team, supporting individuals with Learning Disabilities and/or Autism across Bristol, North Somerset, and South Gloucestershire. Why This Role Matters This pivotal post plays a critical role in shaping and delivering high-quality Continuing Healthcare provision. Youll manage a caseload of highly complex individuals, providing bespoke case management and ensuring their care is safe, sustainable, and person-centred. What Youll Be Doing Deliver specialist assessments and reviews for CHC-funded individuals. Act as a patient advocate for vulnerable clients, focusing on Learning Disabilities and Autism. Develop and implement personalised care plans in collaboration with multidisciplinary teams. Liaise with GPs, social services, community nurses, therapists, care agencies, and mental health services. Conduct Mental Capacity Assessments and lead Best Interest Meetings. Present cases to complex case panels for funding approval. Contribute to policy development and research initiatives within BNSSG. Maintain accurate and up-to-date clinical records. Support audit pathways and budget management for NHS-funded care. What Were Looking For A qualified healthcare professional with experience in clinical case management. Strong clinical reasoning and critical thinking skills. Proven ability to work autonomously and collaboratively across agencies. Passion for improving outcomes for individuals with Learning Disabilities and Autism.
